The patient was managed by a school-based nurse practitioner who implemented a tailored care plan emphasizing culturally relevant dietary counseling, family engagement, and peer support.  Results: Over a 6-month period, the patient demonstrated improved glycemic control (HbA1creduced from 9.2% to 7.5%), increased adherence to blood glucose monitoring, and enhanced self-efficacy in managing his condition. Family involvement was critical in sustaining dietary changes, while peer support reduced stigma and promoted adherence.  Conclusion: This case illustrates the potential of nurse-led, school-based interventions to improve diabetes outcomes in adolescents. Findings underscore the importance of culturally responsive care and family engagement in chronic disease management.
