Glоriа is а 51 yeаr оld female presenting fоr a routine health maintenance visit -- it is the first health visit she has had in almost 12 years. (She has not had a pap or “other stuff that I know I should do” in all this time.) She complains of a constant runny nose almost all year round, and gets lots of “sinus headaches.” Otherwise, she feels well. She has not had a menstrual period for about 3 years. She had 3 children in her 20s and has no major medical illnesses. She takes vitamins only each day, does not smoke, drinks socially and is not sexually active since she separated from her husband 5 years ago. Her PE is completely normal. (Chart a FULL SOAP NOTE)
