She is referred for Outpatient PT, TIW x 4 weeks. Medical diagnosis from workman’s compensation clinic MD: R. shoulder strain/sprain   Imaging: None  PMH / PSH : Breast CA 1/2015 ; s/p R. mastectomy and breast reconstruction 3/2015      SH: bakery worker / cake decorator, single mother, lives alone with 1 teenage child General health: smoked 1 ppd x15 years but quit in 2011; 8 – 10 beers/cocktails a week   Pain: location R. shoulder, sometimes radiates into R. neck and upper back    Patient reports pain wakes her at night, cannot sleep on her R. side. Increased pain with reaching especially out to the side and overhead; difficulty don/doff shirt or coat; blow drying her hair, lifting groceries, laundry basket and cakes / icing buckets in work. Driving is difficult especially moving the steering wheel and turning her head to look over her shoulder. She is currently unable to work because of pain and reports that she is also unable to play in her weekly bowling league.     Pt. goal; Pain free so she can return to work full duty in 6 weeks when short term disability ends.     Review of systems: Denies (answers No) for recent acute illness, fever, nausea, vomiting, bowel/bladder changes, shortness of breath, numbness or tingling.  Reports YES to recent weight loss and fatigue.   Systems Review: Height 5ft 7”; Weight 110lbs; BMI 17.2 A & O x3; primary language English; cognition and communication normal; high school educated  Scars: none visible Posture: Slightly flexed with mild forward head and rounded shoulders   Pain: 4/10 in R. shoulder at rest; 8/10 with attempts to raise right arm overhead Neuro screen: Sensation intact R.
