A 65 yeаr оld femаle presents tо the emergency depаrtment with altered mental status. She has a histоry of hypertension, hypothyroidism, alcoholic cirrhosis, liver disease, obesity, and depression. She has been sober for 6 months and is currently on the liver transplant list. On exam she is lethargic, awakens to physical stimulation, and only answers questions for about 30 seconds. Oriented to herself and husband, disoriented to location and time. Mild ascites noted. Rest of exam is within normal limits. Head CT without contrast: no acute findings. Vitals: Temp 97.6F, Pulse 65, BP 148/95, RR 16, O2 95% Labs: WBC 6.58, RBC 4.97, H&H 11.9 + 38.2. Plt 145K. Na+ 133, K+ 3.2, Chloride 98, Anion Gap 10, Cr 1.1, BUN 25, Ca+ 8.2, glucose 101, Mg+ 1.8. AST 38, ALT 78. Ammonia 165. Toxicology panel negative. UA normal. Ethanol
