He smokes one pack per day (20 cigarettes), but has attempted to quit a few times for a couple of weeks at a time, and once quit for two months. He has no other health issues. His oral self-care includes brushing two times per day.  He does not floss or use any antimicrobial rinses.  At this appointment you updated all the assessments including a full mouth probe (see provided periodontal chart), explored for calculus, and determined he is a deposit one.  The patient’s periodontal pocket readings have been increasing gradually over the last few recalls. You and Dr. Hall review the BWX from October 2024 and determine generalized bone loss with 35% bone loss in the posterior.
