An 85-yeаr-оld pаtient cоmes tо the clinic for а primary care follow-up appointment. She reports no concerns but is largely apathetic and unaware of her medical history. Her responses are tangential and history requires input from her daughter, who reports that the patient's cognition has continued to gradually decline since the last visit. She has been confusing the names of her children and siblings. She requires assistance with dressing and bathing. She has also started to have frequent urinary incontinence over the interval which is sometimes associated with a strong odor and dark colored urine. Medical history is significant for osteoporosis and moderately severe Alzheimer dementia disease. Medications are calcium, vitamin D3, and memantine. Vital signs are normal. Physical exam shows the patient to be alert and oriented to herself only. She is in no distress. There is no suprapubic tenderness or CVA tenderness. On pelvic exam the patient is unable to engage perineal muscles when instructed but there is no obvious sign of prolapse. No urine leakage or vaginal discharge is noted. Vaginal walls are slightly pale and dry. What is the best next step in the management of this patient?
