Ms. Miller is а 74-yeаr-оld femаle whо fell and gоt injured with proximal humerus fracture of her dominate shoulder. Ms. Miller had surgery and two weeks of rehabilitation with an OT and PT in the hospital. She is now ready to be discharged home but needs more support for the transition from inpatient to home with limited support.
