Scene  The pаtient (90 kg) is lying prоne in the drivewаy, which is pаrtially shоveled. There is a shоvel lying next to the patient. A bystander is on scene stating they found him this way while walking their dog. The bystander does not know the patient’s medical history, medications, or allergies.   The patient is rolled into the supine position. He has a patent airway and his breathing is slow and irregular. The radial pulse is bounding and slow. The patient does not open his eyes, only moans to painful stimulus, and withdraws from pain. There are no signs of trauma. The patients’ pupils are unequal; the left pupil is fully dilated and unreactive while the right pupil is 3 mm and reactive to light. Breath sounds are clear bilaterally. The vital signs are BP 192/108, P 56, R 6 and irregular, SPO2 90% on room air, and T 97° F (36° C). The blood glucose is 98 md/dL. The ETCO2 is 63 mmHg.
