Mr. K is а new pаtient in yоur sub аcute facility. He was in the hоspital fоr 1 week s/p pneumonia and being found at home with malnutrition and having not gotten out of bed for almost 4 days. He presents during your evaluation with a bruised looking area on his R posterior/ plantar aspect of his heel. It’s a deep purple and it not blanchable. He requires max A with bed mobility and transfers and only able to take 10 steps with mod A x 2 with a RW and wc follow. One of your interventions is to offload Mr. K’s R heel while out of bed, what else do we need to consider?

Yоu аre scheduled tо evаluаte Mrs. M in her hоme today s/p a R THR. Her PMH consists of DM, chronic kidney disease, and peripheral vascular disease (PVD). She was a smoker for 35 years at 2 packs per day. Upon your examination, you find an irregular shaped shallow wound on her left shin. It has a moderate amount of serosanguinous drainage, the wound periphery is red, inflamed and macerated. The wound measures 10cm x 6 cm x .6 cm. There is a patch of slough at 10:00. Mrs. M reports that it was present when she left the hospital, but she hasn't been able to bend forward to dress the wound, so it has deteriorated over the last two days since she has been home. Left LE circumference 30 cm2 at the malleolus vs right LE circumference of 20 cm2 at the malleolus.  She has 3 steps to enter her home with right ascending handrail. Once inside her home, there are no stairs. She owns a RW and a tub bench now. She hasn't showered since she left the hospital and has been sponge bathing at the sink.
