Cаse Study Pаrt 1 Crystаl is 36 years оld. She sustained a C6 cоmplete spinal cоrd injury when she dove head-first into her swimming pool, resulting in complete quadriplegia. Two months after her injury, Crystal was transferred to a spinal cord rehabilitation program two hours from her home. Crystal has been an inpatient at this rehabilitation centre for the past three weeks. Crystal is happily married and has a 3-year-old daughter. As an occupational therapist you have been asked to see this client to support discharge home. The Client record indicates: · Crystal has limited family support because her husband is a corporate lawyer frequently away on business and her immediate family live in another province. · Crystal was employed as a lawyer in a local office. · Prior to the accident Crystal was very active. She played tennis, sailed and held large dinner parties. · Crystal is currently using the rehabilitation hospital's power wheelchair. · Crystal is independent in bed mobility with a side rail. · Crystal currently requires a mechanical lift to transfer from bed to wheelchair and wheelchair to commode chair. · Intermittent catheterization is done by nursing staff. · Crystal’s husband has extended healthcare benefits through his employer and Crystal is receiving long-term disability funds. The extended healthcare benefit will pay for 20 hours of nursing care a week. Self-Study Exercise Part 2 Read the additional case information below and: 1. Case Study part 2 The findings of your occupational therapy assessment are: Full head and neck movement with good muscle strength and good shoulder movement. Full wrist extension in a gravity eliminated position. Crystal’s medical record indicates she has good skin integrity and no history of urinary tract infection. Crystal is able to feed herself using a feeding strap and fork or spoon during mealtimes. Food needs to be cut. Crystal can sit without assistance or support, propping herself using her upper extremities. Crystal and her husband live in a bungalow home (house with one floor). There are four bedrooms. Outside there are 5 steps leading up to the main entrance. Crystal expresses thoughts of anger focused on the accident and stated she should have "just died". Crystal explains that she frequently has nightmares about body appearance. Crystal wants to sleep through the day and is reluctant to attend her occupational therapy and physiotherapy sessions. Crystal states that she does not have the energy to get up and participate. Crystal is very concerned about losing her daughter if she cannot care for her after completing rehabilitation. She also voiced concerns about her future relationship with her husband. Crystal wants to return home as soon as possible and spend quality time with her daughter. Returning to work is very important to her. Crystal described that she was very spiritual prior to the accident. She practiced meditation and performed yoga several times a week. She is also a vegan (diet with no animal products) and is raising her daughter as a vegan.
