A patient presents to the ED with a history of heart failure for management of symptoms of an acute exacerbation of heart failure.  Work-up notable for the following: Symptoms Dyspnea and non-productive cough Confusion – oriented to self only (normally oriented x4) Physical Exam Findings S3 and S4 gallops on auscultation +3 pitting peripheral edema Extremities cool with prolonged capillary refill time Blood pressure 84/62 Labs / Imaging / Diagnostics Troponin – 0.09 BNP – 965 (baseline ~200) Na – 130 KUB – ” Increased cardiothoracic ratio with bilateral Kerley B lines present” Based on this information, which medication would be the most appropriate first medication to order for this patient?
