A collegiate football lineman sustains an injury from a tackle where his foot was planted, externally rotated, and forced into dorsiflexion. He has significant tenderness over the deltoid ligament and anterolaterally above the ankle joint. Both the squeeze test and external rotation test are positive. This clinical pattern is highly indicative of a syndesmotic injury. What concurrent fracture MUST be ruled out?
