A 68-year-old male client was admitted to the medical/surgical department for fatigue and weakness. The client has a medical history of stage IV lung cancer and had an implanted port placed six months ago for chemotherapy. The nurse just started an IV infusion of 0.9% sodium chloride IV bolus through the client’s implanted port.  The client complains of new-onset shortness of breath after the infusion started. The client’s temperature is 36.9C, respirations 32 breaths/min, heart rate 108 beats/min, blood pressure 146/74 mmHg, and SpO2 is 88% on room air.  Indicate whether the nursing actions listed below are Indicated (appropriate), Contraindicated (could be harmful), or Non-Essential (not necessary) for the client’s care at this time.

When I take proctored assessments, I understand that: Access to the textbook, notes, or any other resources is strictly forbidden. The exam will be timed and must be submitted in one sitting.
