Age  YRS  17 SCENARIO Sex  Mаle      Femаle The pаtient presents with a temperature and swоllen face. Oral examinatiоn indicates bilateral infectiоn posterior to both lower 2nd molars and severe infection in his tongue.  He has tenderness in the mylohyoid muscle under the tongue.   Three weeks ago he had his tongue pierced.  His tongue stud is located in the anterior, central region of the tongue approximately 2.5 cm from the tip.  On the underside of the tongue around the tongue bar and extending to the floor of the mouth is a large swollen tissue that is red and appears inflamed.  The floor of the mouth is elevated.  He has some drooling of saliva at the corners of his mouth.  He complains that the area around the tongue stud is painful.   There is gingival recession and inflammation of the sublingual mucosa adjacent to the lower incisors which you attribute to the stud. Diagnosis:  Ludwig's angina, a serious, potentially life-threatening infection of the tissues of the floor of the mouth.  Height 5’ 11’ Weight 145      LBS B/P 120/65 Chief Complaint Tongue is hurting and have difficulty swallowing. Med History Aortic valvuloplasty at 8 years of age for correction of congenital aortic stenosis. Last saw dentist 5 months ago.
