LTGs and STGs have been established.  Develоp an appropriate treatment plan-interventions-for each Short-Term Goals. Please assign your interventions to the appropriate category as defined in Occupational Therapy Practice FRAMEWORK: Domain and Process, 4th Edition.  Please provide an appropriate justification or rationalization for your interventions. (16 Points) STEWART – OCCUPATIONAL PROFILE AND ESTABLISHED GOALS Student: Stewart Age: 9 years Setting: Elementary School Occupational Profile Stewart is a 9-year-old elementary school student who is experiencing difficulty with classroom performance, play, and social participation. He demonstrates decreased handwriting legibility and has difficulty keeping pace with classmates when completing far-point copying from the board. Stewart also demonstrates difficulty with visual tracking, poor shoulder co-contraction and endurance, hypertonicity, a positive asymmetric tonic neck reflex (ATNR) in standing, and associative reactions when concentrating on academic tasks. Stewart demonstrates sensory-processing concerns, including an aversion to touch and discomfort with movement. These challenges may affect his participation in classroom, physical education, playground, and peer activities. He has difficulty keeping up with his friends on the playground and reports decreased self-esteem when comparing his abilities with those of his peers. Stewart identifies two occupations as particularly meaningful to him: keeping up with his classmates when copying information from the board and playing soccer with his friends during recess. These priorities provide the functional focus for occupational therapy services. Strengths and Motivators Stewart is able to identify and communicate activities that are meaningful to him. He is motivated to improve his school performance and participate more successfully with his peers. His desire to improve far-point copying and play soccer with friends provides meaningful opportunities for occupation-based intervention. Consider intervention activities addressing: Visual tracking and visual-motor performance Far-point copying and handwriting Shoulder co-contraction and upper-extremity endurance Postural control Hypertonicity Sensory responses to touch and movement Gross-motor coordination and motor planning Participation in soccer and playground activities Intervention activities should be age appropriate, meaningful, measurable, and clearly connected to Stewart's established occupational therapy goals. Established Occupational Therapy Goals Long-Term Goal – Academic Year By the end of the academic year, Stewart will demonstrate improved visual-motor integration, postural control, upper-extremity stability/endurance, and sensory-motor regulation to independently participate in classroom and recess occupations, as evidenced by completing far-point copying assignments from the board with at least 85% legibility and accuracy within the allotted classroom time and participating in a 15-minute soccer/recess activity with peers with no more than 1 verbal cue or accommodation, in 4 out of 5 opportunities. Short-Term Goal 1 – First Semester By the end of the first semester, Stewart will demonstrate improved visual tracking, postural stability, and shoulder co-contraction/endurance to copy 3–4 sentences from the classroom board with at least 75% legibility and accuracy while maintaining an appropriate seated posture and requiring no more than 2 verbal or visual cues, in 4 out of 5 opportunities. Occupations and Activities: (2 Points)   Interventions to Support Occupations: (2 Points)   Education and Training: (2 Points)   *Justification or Rationalization for your interventions: (2 Points)   Short-Term Goal 2 – Second Semester By the end of the second semester, Stewart will demonstrate improved motor planning, postural control, movement tolerance, visual tracking, and coordination by participating in a structured soccer or playground activity for at least 10 minutes, including running, kicking, stopping, and changing direction, with no more than 2 verbal cues or accommodations, in 4 out of 5 opportunities.
