A 65 yeаr оld mаn presented tо the ER cоmplаining of severe pain, redness, and swelling in his left knee. His past medical history was significant for coronary artery disease, hypertension, diabetes, and hyperlipidemia. On physical examination, patient was afebrile with normal vital signs and a swollen, reddish, warm, and tender left knee. Arthrocentesis was performed and a synovial fluid was collected. Gram stain, cell counts, culture, and crystal examination of the fluid were ordered. CBC, BMP, PT/INR, PTT, and serum uric acid were also ordered. The synovial fluid analysis revealed the following results: Appearance: Cloudy WBC Count: 20,000 cells/uL Gram stain: No organisms seen Crystals: Rhomboid shaped crystals with positive birefringence under compensated polarized light as show below Image(2).jpg What is the most likely diagnosis ?
