Observational studies may clarify long-term risks associated with nontraditional work schedules. Methods: A prospective cohort study followed 1,200 registered nurses employed at three urban hospitals. Participants were categorized based on baseline work schedule: day shift(n = 600) or rotating/night shift (n = 600). Nurses were followed for 5 years, with annual blood pressure measurements and surveys on lifestyle factors. The primary outcome was incidence of hypertension, defined as systolic ≥140 mmHg, diastolic ≥90 mmHg, or initiation of antihypertensive medication. Cox proportional hazards models adjusted forage, BMI, smoking, and physical activity. Results: Over 5 years, 214 new cases of hypertension were identified. Incidence was higher among rotating/night shift nurses compared to day shift nurses (HR = 1.42, 95% CI: 1.10–1.84). Associations remained significant after adjusting for confounders. Conclusion: Nurses working rotating or night shifts had a higher risk of developing hypertension compared to day-shift nurses. These findings suggest occupational scheduling may be an important modifiable risk factor for cardiovascular health.
