Mr. B.R. is а 65 yr оld blаck mаle with the cоmplaint оf mid-epigastric pain that increases after meals and at bedtime. The pain began several months ago, but is now much worse. Initially the pain was relieved by Tagamet HB 1 tablet with meals and at bedtime, but now he is taking 2 tablets at each dose and the pain still persists. Upon further evaluation by endoscopy he is reported to have a duodenal ulcer and be helicobacter (+) by endoscopic evaluation. He denies recent use of NSAIDS or ASA. He has diabetes and normally checks his blood sugar at home once a day at different times of the day (a.c. or h.s.). He didn't bring his record with him today, because that wasn't why he came to clinic. PMH: diabetes mellitus -- Type II (NIDDM) hypertension (controlled) peripheral neuropathy history of a diabetic foot ulcer -- healed after 6 weeks of treatment with oral cephalexin and metronidazole (antibiotics stopped 2 weeks ago) SH: (+) tobacco 1/2 pack/day; social ETOH use (2-4 drinks on weekends) Allergies: ASA (stomach upset) Medications: metformin -- 1000 mg BID with meals; gabapentin 300 mg in AM and 600 mg at HS for neuropathic pain; hydrochlorothiazide -- 25 mg QD; Keri lotion daily for dry feet -- Apply after bathing, avoid skin between toes. VS: BP 126/76; P 78; Wt. 164 lb; Ht. 5'10" Physical Exam: normal except for some epigastric tenderness and decreased sensation – both feet. Labs: Hct – 44%; Hgb 14.5.
