With respect to any tests/quizzes/exams I take remotely, I hereby voluntarily, and under no duress, consent to the following policies and procedures: Software Installation: I consent to the temporary installation and use of a Chrome extension or other designated software required by the online proctoring service for the duration of the test/exam. I understand this software will be used to monitor my computer and testing environment. System Scan: I consent to a scan of my computer system prior to the test/exam to identify any unauthorized applications, tabs, hardware, or devices that may compromise the integrity of the assessment. Remote Access (Limited): I consent to allow limited, temporary remote access to my computer by the online proctoring service solely for the purpose of addressing technical issues or security concerns that may arise during the test/exam. I understand that the proctor will access my system only under direct supervision and with my awareness. Personal Identification: I consent to taking a photograph of myself for identification purposes. Photo ID Verification: I consent to displaying a valid photo ID to the webcam for verification by the online proctor. Environment Scan: I consent to a 360-degree video scan of the room in which I will be taking the test/exam, including a scan of my testing surface/desk, to ensure a secure testing environment. I understand that I must complete this scan correctly/completely. Video and Audio Recording: I consent to being video and audio recorded during the test/exam via the webcam and microphone on my computer. Screen Recording: I consent to having my computer screen recorded during the test/exam. Data Retention and Access: I understand that the recordings of the tests/exams, along with related information, will be securely retained by the online testing proctor and that Calhoun Community College will have access to such recordings and information for academic integrity purposes. Data Security and Privacy: I understand that the online proctoring service will handle my personal data in accordance with their privacy policy and industry-standard security measures. I have been provided with information on how to access and review the proctoring service's privacy policy.
