She states her headaches “have been a part of my life for the past 10 years.” The symptoms are located on the right temporal side of her head, with frequent bouts of pain around the eye. Occasionally, she experiences pain at the base of her skull. There was no apparent injury when the symptoms first began. Her occupation involves computer data entry. She notices her symptoms worsen after longer days at work. She reports a 6/10 pain level for both the headache and suboccipital pain. Postural observation shows a slightly forward head position. Range of motion reveals flexion to 45°, extension to 60°, and rotation to the right 40° and to the left 65°. Joint mobility assessment indicates stiffness in the right Occiput-C1 joint. She can maintain the cranio-cervical flexion test position (without substitution) for 10 seconds at 24 mm Hg (one repetition). Motion testing of this joint reproduces pain around the eye on the right side.
