This pаtient is а seventy-five yeаr оld man twо days pоst left middle cerebral artery stroke . He has a history of coronary artery disease , heart catheterization twice , hypertension and congestive heart failure . His left side active range of motion is within functional limits and right upper extremity and lower extremity passive range of motion is within normal limits . His right manual muscle testing scores for hip flexion, extension, abduction , adduction, internal rotation and external rotation are equal to a grade of fair while his knee flexion and extension grades are a score of 2 on the standard 5 point scale . He received neuromuscular electrical stimulation to the left vastus medialis oblique to help with repetitions of active assisted range of motion . He was then instructed in a home exercise program of straight leg raises and terminal knee extension exercises, each performed times 10 repetitions, to be performed twice a day without the extension brace, followed by a cold pack to the knee for pain as needed . He also received gait training to the bedside commode with a large base quad cane and maximum assistance of one person for a distance of approximately 15 feet from his bed.
