Histоry оf Present Illness (HPI): The pаtient received а diаgnоsis of Parkinson’s disease. Her neurologist has sent her to physical therapy for evaluation and management of the symptoms of her Parkinson’s disease. She states that she noticed she was walking slower when she could not keep up with her husband on their 30-45 min evening walks. Additionally, her husband remarked that she did not swing her left arm when walking. She is a retired administrative assistant for a pest control company and her husband owns a landscaping business. She and her husband enjoy reading, going to the movies and evening walks. She keeps a tissue in her right hand and tabs her lips every 2 to 3 minutes. She is considered to be in Hoehn and Yahr (H&Y) stage 1 and scored a 10 on the MDS-UPDRS (Unified Parkinson’s Disease Rating Scale). Pt’s Goals: Manage Parkinson's and maintain independence as long as possible. Past Medical History (PMH) COPD x7 years, well controlled with medication. Gallbladder removal at age 42, mild OA B Knees. Two c-sections, with each child (2). Medications: Centrum Silver multivitamin daily Advair Diskus (250/50) twice daily Tylenol 500 mg 4 times daily as needed for pain Home Environment: She lives with her husband and 2 French bulldogs in a small single-story home, and her son lives with his family 3 hours away. She has smoked one pack per week for 30 years but quit 5 years ago and began a regular walking program with her husband for exercise. She drinks alcohol socially on holidays. She does light housework daily in her home and leaves the heavy cleaning to her husband.
