Jаnet Miller is а custоdiаn, whо wоrks full time at a large school district in San Luis Obispo, California. She is 42 years old and has been working at the school district for eleven years. On September 29, 2024, while working her usual and customary duties at her assigned school site, she slipped and fell on the tile floor she was mopping. She landed on her right side on the tile floor. She began to feel immediate pain to her right hip. She was able to get up from the floor on her own and completed her shift without incident. There were no witnesses to the fall. The next day, September 30, 2024, she stayed home as her right hip was aching and bruised. She called her supervisor on September 30, 2024 and told him about the fall at the school site and that she was having pain in her right hip, which was bruised. Her supervisor instructed her to fill out a claim form, sign and return it, and she was also instructed to go to the industrial clinic near the school site. At the clinic, she told the attending physician she had fallen on September 30, 2024, explained the mechanism of injury, and advised him she felt she had injured her right hip due to the fall. In his report of September 30, 2024, the industrial clinic physician ordered x-rays of the right hip, provided medication, and provided work restrictions to the employer of no lifting above 10 pounds, no repetitive pushing or pulling, no climbing stairs, and no walking on uneven surfaces. If these work restrictions were not able to be accommodated by the employer, she would be temporarily totally disabled. The industrial clinic physician diagnosed right hip contusion and stated the causation of the right hip injury was due to the fall. She returned the claim form to the employer on October 1, 2024 stating she fell on September 29, 2024 and injured her right hip while she was at work. Janet Miller runs marathons on a yearly basis.
