A 35-yeаr-оld wоmаn underwent multiple surgeries аfter a car accident. Her surgeоn prescribed 10 mg of Vicodin (acetaminophen+ hydrocodone) taken daily for her postoperative pain. The treatment provided adequate analgesia (pain relief). The accident led to a recurrence of her major depressive disorder, for which she renewed her use of anti-depressant fluoxetine (Prozac) at 20 mg/day. She was unaware that hydrocodone is an inactive prodrug and the substrate for CYP450-2D6 enzyme that transforms it to hydromorphone – an active form of this drug and the actual analgesic. She was also unaware that Fluoxetine is a strong CYP450-2D6 inhibitor. As result of this drug interaction she experienced:
