Assessment There аre 2 pаrts. In pаrt A yоu will analyze the case example test results (given in Appendix A) tо derive a differential diagnоsis. In part B you will answer related questions to show your knowledge about the different assessment tests and protocols. Write your answers underneath each sub-question. Part A – Analyze a case example COMPLETE CALCULATIONS – give the values for these remaining metrics based on the results given to you. Audiogram: Calculate the pure-tone averages (PTAs – show your work. You can use the computer calculator for the final calculation). Write what you expect the speech recognition thresholds (SRTs) to be based on the hearing results. Tympanogram: Give the approximate acoustic immittance values below for both ears by interpreting the tympanograms given to you. Equivalent volume (ml) Middle ear pressure (daPa) Static admittance (ml) INTERPRET TEST RESULTS – Interpret each of these test results to indicate if they are normal or abnormal, and explain your answer (ie why normal or why abnormal). You are given normative values for immittance and ABR to support your answers Audiometry: Describe the type and degree of hearing loss in both ears. Acoustic reflexes Speech audiometry DPOAEs ABR DIFFERENTIAL DIAGNOSIS – Using the information you provided in the previous questions (the test results), as well as the normative values provided to you, give a possible diagnosis and support your answer with details by integrating information from the tests above. Your answer, with justification, should be at least a few sentences long. Part B - Questions related to the case example MASKING Give the approximate interaural attenuation for insert earphones, used in this testing. Describe the initial masking level that you would have used for 4 kHz bone conduction in the left ear and explain how you would determine when you have found your masked threshold. Explain why air conduction masking was not necessary at 4 kHz. If you had used TDH headphones, would you need to mask air conduction at any additional or fewer frequencies, why or why not? ABR PARAMETERS Describe the likely parameters that were used to record these click ABRs. Include the typical ABR montage, level, and stimulation rate (i.e., what overall parameters are used to best visualize each wave of the ABR for differential diagnosis?). Explain why we don’t see wave I in the contralateral channel. RELEVANT TESTS Would running an EcochG test give you additional information that would help you decide your differential diagnosis? Explain why or why not. OTHER TESTS Describe the setup for a pure-tone stenger test for this patient, and what we would expect in terms of a behavioral response for a positive stenger test, and a negative stenger test.
