Per EMS, the patient’s husband is parking the car and will be in shortly. History and Physical The patient is a 63-year-old, African-American woman with a history of Diabetes Mellitus type II, hypertension, and rheumatoid arthritis. She works as an attorney in a busy office and lives at home with her husband. Due to aphasia, her husband is answering questions for her at the bedside. According to the patient’s husband, the patient has been having some increased stress recently related to work and to his knowledge she has not been monitoring her blood pressure. He is able to describe the happenings of the morning and the night prior. He states that the patient was in her usual health the night prior and went to bed around 2200. As per their routine, they woke around 0500, the patient walked her dog and came back inside. As she was walking past the coffee pot, she poured 2 cups of coffee and carried one in each hand to the table and sat with her husband to watch the 6:00 news. She was able to converse normally with her husband and he did not notice any slurred speech or facial drooping. At approximately 0630 she went to her vanity, as she normally does, to apply makeup and get ready for work when she called out to her husband. While attempting to apply makeup she noticed that she wasn’t able to hold her makeup brush with her right hand. Her husband came to her side and notice that her face was drooping. Noticing that this was a sign of stroke, her husband call 911 and she was transported to the Emergency Department.
