In verifying this you acknowledge that your failure to adhere to or follow the above requirements can reflect a deliberate intention to cheat on this test and can be grounds for you to be assigned a 0 grade and reported to the chairperson of the Arts & Sciences Academic Integrity Committee for further disciplinary consideration and action.
